Requirements for replacement of lost passports

If you have lost your passport and are applying for a new one, you MUST personally appear at the Embassy with the following:

  1. A Police report of the incident
  2. SwornAffidavit Form.
  3. Declaration Formfor Ugandans living abroad.
  4. Citizen Verification Form
  5. CompletedForm Aand explanation of loss of passport
  6. CompletedQuestionnaire
  7. Two (2) clear passport size photographs.
  8. A money order of $180.00 non refundable processing fee.
